We also collect information when you voluntarily complete customer consultation forms, customer surveys and provide feedback. Website usage information is collected using cookies. Most of the personal information we process is provided to us directly by you for one of the following reasons:

· To complete consultation form prior to treatment/services you have booked to ensure there are no health contraindications to treatment/services and to hold up-to-date information regarding your treatment/service requirements.
We use the information that you have given us in order to contact you about treatments/services you have enquired about or have booked and to keep an up-to-date client record to enable safe delivery of treatment/services.
We do not share your information with any third parties, unless legally required to do so, you ask us to, or in order to comply with safeguarding requirements set out by law.
Under the UK General Data Protection Regulation (UK GDPR), the lawful basis we rely on for processing this information are:

Data Retention - How we store your personal information
We keep your contact details and personal information provided on paper consultation forms that you have completed for a minimum of 10 years as required by our professional membership with the Federation of Holistic Therapists and our insurance provider Hiscox. These paper forms are securely stored in locked storage. We will then securely destroy paper forms containing this information by shredding and disposing of in secure confidential waste.
Any information about you that is received via email or contact forms is kept in a secure email filing system which requires a password and security code to access. This information is only held by Helen Elizabeth Therapies & Wellbeing. This information will be retained for 10 years and securely deleted after this time.
